Vendor Registration Application

⏰ Event Time: 10:00 AM – 3:00 PM

🛠 Vendor Set-Up: 8:00 AM (Prompt arrival required – NO EXCEPTIONS)

SET-UP INFORMATION

• One (1) table and two (2) chairs included
• Vendors must provide additional equipment (tents, extension cords, extra tables/chairs)
• Vendors must arrive by the designated set-up time

VENDOR AGREEMENT

By completing the form below, Vendor agrees:

• All fees are due at registration
• All payments are non-refundable
• Vendor is responsible for setup, sales, and cleanup
• Vendor will comply with all event rules and local regulations

Liability: Vendor agrees to indemnify and hold harmless Mount Moriah Missionary Baptist Church, its officers, agents, and volunteers from any claims, damages, or losses arising from participation. Organizer assumes no responsibility for theft, loss, or damage to Vendor property.
